Regularly, people ask us as their GP, what care they can get and choose in the last stage of life, even before there is any illness. As a consultation is too short to give all the information, we would like to invite you to a information meeting at Thursday 13 February from 17:00 to about 18:00. The meeting is at Het Klokhuis, Celebesstraat 4, 2585 TJ The Hague.
During the meeting on the last stage of life, you will get information on:
- What treatment choices you can make.
- What care choices you can make.
- How to record your wishes, where and with whom.

Check with your GP or pharmacy whether they offer MijnGezondheid.net. To complement the MijnGezondheid.net patient portal, you can use the MedGemak app.

Your (GP) doctor and pharmacies can connect their computers to the National Switch Point (LSP). Through the LSP, they can share your most important medical data. Another healthcare provider can view that medical data.
